It is a social construct. It is not natural to have a feminine appearance and a penis and to want to model in porn. However, the term shemale is offensive and should be avoided.

Now if you mean actual transsexualism, a person might be born trapped in the wrong body, and then they take hormones and get surgery to fix that.
