The answer is probably better phrased as "should" rather than "have to".

Most health insurance policies have deductible and copayment provisions. One of the best ways to keep track of your financial responsibility as the insured, and the insurer's financial responsibility, is to file all claims (or to make sure that the health care providers do). The insurer will then process them and issue Explanations of Benefits (EOB), which will show what was paid, what was not paid, and where you stand with regard to meeting deductibles and copayments for the period of time in question.
