The reason we gargle with salt water for a sore throat is for both a cleansing action and to slightly reduce swelling and inflammation in the throat. Both of these properties of cleaning and reducing swelling come from the salt itself. Using something sweet in the water such as sugar would take out the element (salt) that is helpful in this case, so it would not be as effective as salt water.
