How do you replace your ripped citizen certificate?

If your certificate of citizenship is lost, damaged or mutilated then you need to file Form N-565 to replace it with a new one. You need to submit the damaged certificate also along with Form N-565. Two passport sized photographs ( recently taken ) should also be attached.
