Most healthplans maintain a list of preventative care services that are covered by the plan. Each plan has a document called an Evidence of Coverage and/or Summary of Benefits which should outline their definition of what they cover. The National Plans base their recommendations on Independent National Health Care Organizations like the American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP), American Cancer Society (ACS), etc who make certain recommendations for various tests that should be performed at a certain age, over a certain period of time, for early detection and preventation of certain diseases or health issues. For example:

Breast cancer Screenings

Cholestrol Screenings

Colorectal Cancer Screenings

Osteoporosis Screening

Cervical cancer Screening

Immunizations for Children as recommended by the Centers for Disease Control
