Get a Cover Letter and fingerprint cards from the FBI website.
Go to the local police station and get the fingerprints taken (this can also be achieved by identifying a fingerprint verification company who can do it).
Include credit card form (18USD) (the form is available in the FBI website) and identity proof.
The local police station told us that we need to post it ourselves.
Included a return envelope with postage paid and posted it to FBI.
