If you're talking about a police report, you'll need to provide the officer with your name and insurance information, as well as the date, time and location of the accident. Be sure to also include any information you have on the other driver (name, insurance company and policy number, vehicle make/model and plate #). There's no guarantee that the police will even take the report if it wasn't reported right away, but you can try.
