When must a supervisor complete an accident record?

The supervisor must complete an accident report as soon as possible after an accident occurs. Each company has its own minimum requirement for when a report is triggered. Most require a report if there is an injury requiring medical treatment or first aid and some require reports for for any injury or event that might have resulted in injury but for luck.
