How do you write a excuse note to a teacher to delay a due date of your project?

Firstly, you say 'To whom it may concern' or if you know the teachers name write that. Secondly, if the school has id numbers for the students u must write that along with the date and why. SIgn it, then put your name. It MUST be in pen as it is a legal document. It should look something like this. Dear Miss/Mr......../To whom it may concern Please excuse (childs name), id number, for being away Tuesday, 17th, June 2008, because she was at home sick. (signature) (your name) There you have it!!
