The top of the foot has a nerve/muscle attached to the foot bones to flex the foot. You notice a slight slope dropping down to the outside of either foot from the nerve/muscle top. When you wear your shoes, no matter how tight you lace your shoes, the flexing of the foot causes the shoe tongue to "slide down", angling the tongue to the outside of the shoe.
