Why couldn't the US create or sustain a democracy or republic in the 19th century without civil war?

Because the newly-rich Southern planters dreamed that they were Knights of Chivalry, living on their feudal estates - while the Northerners were building the most modern democracy on earth, helped by new immigrants with industrial skills.
