Do you put a colon after the word by on a title page?

No.

"Written by: Bob Smith" is incorrect.

The colon does NOT belong.

"Written by Bob Smith" is correct.

This is true in every instance - on a title page, on the cover of a book, in a paragraph... There are no cases where a colon belongs after the Word "by."
