If a Honda Accord has OBD code P0420 in the catalyst bank is it the oxygen sensor or the catalytic converter?

There are a number of things that could be causing the P0420 catalyst efficiency code. The most common thing is the catalytic converter is no longer working. The other thing is the rear oxygen sensor is no longer working properly or there is a leak in the exhaust system.
