I'm going to get the middle of my ear pierced twice and am wondering how it's charged I'm going to Claire's and will they'll add a extra fee Does it cost more since it's not the lobe of the ear?

Claire's does not pierce in inner parts of the ear. They will only pierce the ear lobe as they do not have the right type of needles.
