TNKase (tenecteplase) should not be given post cardiac arrest if the patient has a known or suspected active bleeding, a history of hemorrhagic stroke, or if they have had a recent major surgery or trauma. It is also contraindicated in patients with severe uncontrolled hypertension. Additionally, TNKase should not be administered if there is a clear indication that the arrest was not due to a STEMI or other condition that would benefit from thrombolysis.
