It really depends on the animal. Fishes have gills, which help them breathe the oxygen dissolved in water (whereas we breathe the oxygen in the air). They also eat different food than us, like other creatures or coral reefs. Other marine animals (such as marine mammals and amphibious reptiles) breathe air and must surface regularly to take a (very large) breath.
