Frequently because they are not robustly reviewed. When a development plan is created many expect its content to be a straight line to success - No so. A Development plan is set out as a map to get from A to B. When such a plan is created all parties should have input and understand what their task is to help all teams get from the concept of the plan to the end of the plan.
Each section of the plan (task) will have an owner and a target date to achieve. When the plan goes live one person should be responsible for collating where everyone involved is up to in its progress. If one section hits a stumbling point which may be unforeseen then representatives from each time (task force) will meet to discuss what has occurred and how this can be overcome, moved or steered around without inflicting burden on other s as far as it is possible. These are review meetings which MUST occur regularly. With regular meetings taking place and open, honest frank discussions occurring many plans will and do succeed. The greatest cause of failure is the management of the plan and those with the authority to do so not taking responsibility for driving it forward
