I am a newly qualified nurse who has worked in Ghana, so my experience is limited. I have been researching into this exact question with regards to circulatory shock. Many books give information on skin colour becoming pale & greyish, but information on brown skin is scarce. When I was in Ghana I saw a number of patients go into critical condition (sometimes circulatory shock) and there was a definite change in skin colour. They didn't go a lighter shade of brown but it appeared as if there was a chalky white tone to the existing colour. & finally I'm a light skinned black man myself and so have definitely seen a paler version of me in the mirror when I've been very ill.
I'm in the middle of research so i hope to soon be able to provide a more definitive answer.
