It will depend upon the type of surgery. If it was a small hernia it should settle in about six weeks. A large hernia going down to the scrotum, may have large amount of haematoma and it may take up to 3 months. Swelling in the testis can be due to infection

have a scan to confirm / exclude this and you need to see your Doctor. In a large inguinoscrotal hernia part of the sac is left with the testis and if this is the case, the testis may always be slightly bigger than the other side.
