How do you parenthetically notate a website?

You write the main theme, or part of the website's information. You then write the date that it was last modified or the date that you visited it. Lastly, you write the name of the website.

Example: "Lewis and Clark in North Dakota." 15 Jan 2005. www.state.nd.us/hist/LewisClark/indexFrameset.html>
