You have to get a prescription for your health care provider, along with a letter of medical necessity. Also, you will most likely get rejected at first, then appeal it under the fact that your medical booklet does not say that it isn't covered, but they do cover prognostics. Also, if they say that your provider is "not eligible" then state that it is the closest one to you, and that their are no local providers for this. This had worked with several of my previous insurance companies, but unfortunately my employers switched providers.
