How do you open the massive doors in Dragon Age Origins Lost in Dreams on Xbox 360?

At various places in the fade you'll meet people who can teach you how to change into a new form, including a mouse (stealth) and a burning man (can cross flaming barriers). One other form is the golem, and that one is required to open the massive doors.
