Why is it necessary for patients on anticoagulation therapy to be closely monitored?

Anticoagulants decrease clotting time in the bloodstream. Labs are drawn and sometimes stool is tested for blood to make sure there isn't a bleed that is not visible, or to make sure a patient isn't at increased risk for bleeding.
