What harm can be done to a manual transmission small car if started in 2nd gear from dead stop?

Transmission probably not hurt but it is hard on the clutch and can really strain the engine. If the driver didn't know what he/she was doing, it MIGHT be a little hard on the clutch, but it wouldn't really make much difference even then. In othe Words, no problem.
