How do you write application for cancellation of lost demand draft?

You have to write an application in favour of Branch manage of your SBI Branch. In case you have photocopy of DD attached with the application. They will refund to the bank account only. However in this process they will refund only after monhs when they will realise that DD have not been used (3 months). They can ask you copy of the written complaint to the police.
