How do you hand write a poems title?

Use title case.

i.e. Use capitals for the first letter of each Word, except for definite articles, indefinite articles, some conjunctions and prepositions, although there is no standardised form for title case. If in doubt, check other authors' usages and make an aesthetic decision.
