When do you unlock blue baby in the binding of issac?

In "The Binding of Isaac," you unlock Blue Baby by defeating Mom's Heart for the first time. This unlocks the character, who is also known as "???" and starts with unique items like the Poop and a set of soul hearts. After unlocking, Blue Baby can be selected in future runs.
