Both procedures are considered elective and therefore not covered by most insurance plans. Fourteen states mandate that insurance plans either offer or cover certain infertility treatments such as IVF, and typically only the insurance plans subject to the mandate provide any infertility related coverage.
If you work in one of the fourteen states with such a mandate, research the laws, and understand the loopholes. Many mandates apply only to employer groups of certain sizes. Others exempt self insured employers.
If you work for an employer headquartered out of state, research the laws for your company's home state. Your insurance plan will be subject to the laws in that state.
