I have been suffering from a severe tooth ache for a few days. I had been to the doctor last week and my treatment is going on. Regarding the same I have got an appointment with my doctor on coming Tuesday at 1.30pm.

I wish to apply for a half day on Tuesday ie. 17th of April'10.

I have attached my earlier medical reports for your easy reference.
