How does a name get the suffix the third?

In some families a male child is name after his father (using the entire name)

Father - John Herbert Alfred Jones

Son - John Herbert Alfred Jones Jr. (the second)

Grandson - John Herbert Alfred Jones III (the third)

This naming system is most common in male offspring as female offspring tend to marry and change their names, thus breaking the line.
