Can I take my outdoor plants that I planted with me when I move out of my rented house?

I'm not a lawyer but it depends on the contract you signed. Usually you will have signed to leave the property in as good of condition as when you came, with normal wear and tear.

If you have to dig up your plants you will need to fill in the holes and maybe replace the sod or grass.
