Ideally a horse should be re-shod every 6-8 weeks but it depends on the horse in question, the rate of hoof growth, how often the horse is worked (especially on hard surfaces) and if your horse is prone to shoe-throwing (like mine is :P).

The easiest way to tell is to constantly check the hoof and shoe. If the shoe is coming up shiny on the exposed surface have your horse reshod as the shoe will now be too slippy to be safe. Also check the nails - make sure they aren't sticking out or being grown over. Also, if a hoof continues to grow it may grow around the shoe. All of this being taken into account, it is generally safer (and easier on the owner) just to have the horse re-shod every 6-8 weeks.
