One reason the Alma Ata vision of primary health care did not gain more traction in the 1980s and 1990s was the lack of political will and commitment from many governments to implement its comprehensive approach. Additionally, the focus on vertical health programs, such as those targeting specific diseases, diverted attention and resources away from strengthening primary health care systems. Economic challenges and structural adjustment policies in many countries also prioritized cost-cutting measures over health care expansion, undermining the principles of Alma Ata.
