How do you know if someone is really listening to what you have to say?

Ask them questions throughout.

For example if you were saying "the temperature of the Apple is is 45 degrees Celsius when it is lowered into the vat and smothered in chocolate, this is because the chocolate is warm. Tom, what is the temperature of the Apple?"
