First, sign in to Shockwave UNLIMITED with your user name and passWord to gain access to your profile. Click the "My Shockwave" tab at the top of the page. Then, click on the "Account Settings" button. Finally, click on the "Cancel" button underneath the heading "Shockwave Unlimited Status".
