The very worst hindrance in communication, regardless of the form that the communication takes, is understanding. If the listener or reader does not understand what is being communicated, the effort is a total failure and the one attempting the communication must try other ways of expressing the intended ideas.
Perhaps the second-worst hindrance is that the intended audience does not receive the communication for various reasons whether due to inattention, lack of access to the method of communication being attempted (for example, if I were to call you, but your phone battery was totally dead, you would not be able to hear what I had to say and you'd have to call back after your phone was charged), or the message is lost in the mail or a pile of papers on a messy desk, etc. I say that this is second-worst because if the communication is eventually received by the intended audience all is well as long as the person could understand it.
Other hindrances that you may see listed out there are usually another way of putting the first two types, or are closely related to them such as: language barrier, cultural differences, competition for the listener's attention, personal feelings that one or both parties attempting the communication might have for the other (for example, it is very difficult to listen to someone and really understand what they have to say if you are distracted by being angry with them), poor writing/ or speaking technique, etc.
