In the U.S. most insurance companies will not cover the cost, while they almost always cover the cost of the initial vasectomy. Both procedures are elective. One is typically covered and one is not. Just follow the money to understand why.

A vasectomy costs the insurance company a small amount compared to the pregnancies it prevents. Normal delivery costs more than a vasectomy, a complicated pregnancy costs even more, and costs for a premature delivery go through the roof. It makes good business sense to cover this elective procedure.

A vasectomy reversal puts the insurance company back on the hook for a possible normal delivery, complicated pregnancy, and/or premature birth. It does not make business sense to pay for this procedure. Therefore, most insurance plans don't cover vasectomy reversals.
